SUMMARY
Under general supervision, is responsible for devoting their attention to the care and development of the people we support in one of the Agency’s residential facilities. Interacts in a manner which fosters dignity and learning in a way that helps lead to greater independence. Helps create a homelike atmosphere for the people we support in an agency facility.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following:

<li>Provides care and assists with personal development and independence of the people we support.</li>
<li>Ensures that all measures for safety and well-being are maintained at highest level.</li>
<li>Provides training for the people we support in activities of daily living and in development of self-help and social skills.</li>
<li>Maintains a personalized, homelike atmosphere in the residential facility.</li>
<li>Attends weekly staff meetings as well as any other assigned meetings and in-service trainings.</li>
<li>Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned.</li>
<li>Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action.</li>
<li>Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures.</li>
<li>Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices.</li>
<li>Summons police, fire, ambulance or other emergency personnel if situation warrants it.</li>
<li>If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines.</li>
<li>Provides transportation for the people we support in agency vehicles to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc.</li>
<li>Assists with meal planning and preparation as well as with purchasing of food and supplies.</li>
<li>Demonstrates an understanding of and implements behavioral programs.</li>
<li>Participates fully as a member of interdisciplinary plan.</li>
<li>Maintains an ongoing informal exchange of information. Maintains written logs for recording daily activities.</li>
<li>Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a safe and homelike environment.</li>
<li>Supervises any other activities as may be necessary to maintain quality of the program.</li>
<li>Dispenses prescribed medications to the people we support after successful completion of the medication administration course, and in line with Agency protocol. AMAP’s are supervised by residential nurse in all areas related to medication administration.</li>
<li>Completes all activities related to skill buildings as outlined in individual program plans</li>
<li>Provides support around personal hygiene, i.e. toileting, bathing and changing as needed.</li>
<li>Handles all personal documents of the people we support in a secure and confidential manner.</li>
<li>Performs related duties as requested.</li>

<li>Report any sign or symptoms of illness or adverse medication reaction to the site supervisor to implement an appropriate plan of action.</li>
<li>Assists in the resolution of crisis management and reports all incidents, both verbally and in writing to the program supervisor.</li>

<li>Ensures that all individuals attend activities daily as agreed upon, within a structured treatment program, sheltered work center, training program or competitive employment in the community.</li>

<li>Rate of Pay: $85,000–$105,000 annually (Exempt – Salaried) + 10% annual bonus</li>

What You’ll Do:
Clinical Leadership &amp; Compliance
<ul>
<li>Ensure compliance with all federal, state, and local regulations, as well as company policies and ownership standards</li>
<li>Oversee assessments, service plans, and resident health documentation in accordance with state requirements</li>
<li>Monitor clinical outcomes, implement corrective action, and lead quality improvement initiatives</li>
<li>Partner with the Executive Director and department heads to align resident health and safety goals with community operations</li>
<li>Maintain open communication with residents and families regarding changes in care or health status</li>
</ul>
Resident Care &amp; Services
<ul>
<li>Lead the clinical team in providing exceptional care, including wellness checks, medication management, and service plan updates</li>
<li>Ensure resident service plans are accurate and updated to reflect current health status</li>
<li>Partner with physicians, pharmacy consultants, and allied providers to ensure continuity of care</li>
<li>Support education and engagement of residents and families in the care process</li>
</ul>
Team Leadership &amp; Development
<ul>
<li>Hire, train, schedule, and supervise nurses, care managers, and other clinical team members</li>
<li>Provide ongoing coaching, feedback, and professional development opportunities for staff</li>
<li>Foster teamwork, accountability, and a culture of compassion and service excellence</li>
<li>Ensure adequate staffing levels, competency training, and clinical supplies to meet resident needs</li>
</ul>
Qualifications:
<ul>
<li>Registered Nurse (RN) or LPN license in good standing in the current state required</li>
<li>Minimum of one (1) year of nursing leadership experience </li>
<li>Experience managing clinical teams, regulatory compliance, and care planning processes</li>
<li>BLS Certification required</li>
<li>Prior leadership in Assisted Living or Memory Care preferred</li>
<li>Regulatory Knowledge: NYS Department of Health (DOH) regulations preferred</li>
<li>Strong leadership skills with the ability to motivate, develop, and manage a clinical team</li>
<li>Excellent communication, organizational, and problem-solving abilities</li>
<li>Proven ability to manage multiple priorities in a fast-paced environment</li>
<li>Proficiency with Microsoft Office and electronic health record systems</li>
<li>Participation in on-call schedule required including Manager on Duty (MOD) coverage </li>

We are seeking a creative, compassionate, hands-on Lifestyle Coordinator to assist in presenting and coordinating recreational programs for our assisted living residents, as well as residents with Alzheimer’s disease or dementia in our dedicated Reflections Memory Care Neighborhood. You will assist residents in making the adjustment to a new concept of living at our luxury assisted living community, not only encouraging them to continue pursuing their interests, hobbies and lifestyle activities, but also to develop new and stimulating interests, as well. You will help plan fun and safe trips to suitable destinations, such as art galleries, theaters, music venues, racetracks, sports events as well as religious and cultural programs. You will also help prepare, print and distribute copies of our monthly calendar, newsletters and memos to residents. You will also be expected to perform planned activities in the absence of the Director of Recreation. In this role you will maintain a knowledge of resident backgrounds, hobbies, special interests, and spoken languages, etc., and use this knowledge to encourage resident engagement and participation. The loving attention you bring to the assistance you provide is fundamental to the quality of life and the general wellbeing of our residents every day.
The ideal candidate will have the following skills/qualifications:

<li>One year’s prior experience working with a senior population, and those with Alzheimer’s disease and dementia a plus</li>
<li>At least 6 months of Recreation activity experience (art therapy, music therapy) or event leadership experience, and preferably in assisted living, long-term care or a similar industry setting</li>
<li>Be outgoing, warm and have a genuine caring personality</li>
<li>Able to create programs that get residents involved and excited</li>
<li>Be dependable and able to supervise activities, special events and safe excursions for the residents</li>
<li>Has creative talents in one or more areas, such as art, music, readings, dance, etc.</li>
<li>Always maintaining oneself well-groomed and properly attired</li>
<li>Demonstrates good listening and communication skills and always presents a positive spirit and professional attitude</li>
<li>Possesses a proven ability to lead classes and other instructional learning sessions and speak in front of a group setting with ease, confidence and clarity</li>
<li>Holds a high school diploma or equivalent at a minimum</li>
