A-Plus Care Home Care Agency is seeking a Bilingual Case Coordinator to join our dynamic and compassionate team. If you’re a detail-oriented, energetic individual with excellent communication skills and the ability to juggle multiple tasks in a fast-paced environment, we want to hear from you!

As a Case Coordinator, you will play a crucial role in ensuring the smooth coordination of cases and services for our patients, all while delivering outstanding customer service. You’ll assist in scheduling, managing communications, and working closely with both patients and team members to make a real impact on people’s lives.

Key Responsibilities:

  • Case Scheduling: Assist in coordinating and scheduling patient cases to ensure smooth transitions and timely care delivery.
  • Communication: Manage multi-line phone systems while handling calls from patients, caregivers, and staff efficiently.
  • Customer Service: Provide exceptional customer service to patients, caregivers, and team members, ensuring positive interactions at all times.
  • Multitasking: Thrive in a fast-paced environment, juggling various tasks simultaneously, with attention to detail and accuracy.
  • Teamwork: Work closely with co-workers to ensure the best outcomes for patients and support the operations of the agency.
  • Software Management: Utilize HHA Exchange software and Microsoft Office to manage patient information, schedules, and communications.

Qualifications:

  • Bilingual: Must be fluent in both English and Spanish (only apply if you can speak both).
  • Experience: Previous experience in a healthcare or home care setting preferred.
  • Skills: Strong organizational, communication, and multitasking skills. Must be highly motivated, flexible, and able to work well under pressure.
  • Customer Service: Excellent interpersonal and customer service skills with a compassionate approach to patients and families.
  • Computer Skills: Proficient in Microsoft Office and experienced with HHA Exchange software (or willing to learn).
  • Energetic & Positive: Must be self-motivated, energetic, and able to remain calm and positive even in stressful situations.

Job Type: Full-time

Pay: $21.00 - $26.00 per hour

  • Monday to Friday
  • Rotating weekends
  • Weekends as needed
