Position Summary:
The Nurse Manager of Case Management at an esteemed medical center within a world-renowned hospital network is responsible for overseeing comprehensive care coordination. This leadership role ensures high-quality patient outcomes and optimal resource utilization through effective care path facilitation, data collection, and management of both general and high-risk patient populations. The Nurse Manager applies system-wide criteria to guide interventions, assign staff, and address coordination challenges.
Key Responsibilities:
- Lead the case management team, ensuring optimal coordination across inpatient, outpatient, and home health settings.
- Apply utilization and case management principles to manage patient care effectively.
- Maintain expertise in managed care, performance improvement, population health, transition planning, and payer requirements.
- Oversee interventions, staff assignments, and resource utilization, ensuring alignment with best practices and regulatory standards.
- Serve as a key liaison between clinical teams, administration, and external stakeholders to support seamless care transitions.
- Foster a collaborative and supportive work environment while managing a team of professionals, including unionized nursing staff.
Qualifications & Requirements:
Education & Credentials:
- Bachelor of Science in Nursing (BSN) – Required
- Master’s in Nursing or a related health profession – a Plus
Experience:
- 3-5 years of experience in Utilization Review, Discharge Planning, and Case Management – Required
- 1+ year of management experience – Required
- Experience in acute care or hospital system – Preferred
- Familiarity with union environments and collective bargaining – Preferred
- Pediatric experience – a plus
- Proven ability to apply utilization and care management principles in a healthcare setting.
- Strong leadership, communication, and assessment skills, with excellent judgment in clinical decision-making.
