Position Description

Under the direction of the Director of Patient Services (DPS), the LPN performs routine nursing activities in the care and treatment of clients in their home in accordance with an established care plan provided by the patient's Case Manager RN. LPN will follow the established care plan to meet the medical needs of the client. Must establish a good rapport and effective means of communication with clients of varying abilities. Participates in a team approach to providing client care and as such must establish good working relationships with co-workers.


Essential Duties and Responsibilities

  • Assists with the preparation, implementation and continuing evaluation of the nursing care plan.
  • Communicates with the physician as directed by the RN Case Manager and DPS.
  • Obtains interim physician orders as appropriate and notifies the RN Case Manager prior to carrying them out.
  • Carries out therapeutic treatments such as complete bedside nursing care, treatment of wounds using a variety of applications, skin care, preparing and applying dressings, and preparing and administering medications.
  • Carries out advanced clinical skills for which the LPN has received training and demonstrated competencies. Assists with the rehabilitation of clients in accordance with the nursing care plan, such as assisting the client with ambulation, assisting clients with normal range of joint motion, and applying the principles of good body mechanics and body alignment.
  • Observes, records and reports progress or change in the client's condition as well as questions and concerns to the RN Case Manager and DPS.
  • Competes in service trainings as required.

Other Responsibilities:

  • Documents the services provided and observations made; submits reports in a timely manner. Maintains essential nursing records and reports.
  • Must follow infection and exposure control protocols.
  • Meets with DPS and RN Case Manager during scheduled times.
  • Attends required agency and departmental staff meetings.
  • Complies with agency policies and procedures.
  • Follows the Code of Conduct that must be reviewed and signed on an annual basis.
  • Undertakes special activities as assigned and other duties as appropriate.


Physical Requirements:

  • Physical demands may include but are not limited to lifting, bending, standing and transferring patients (with equipment when necessary).
  • Home care field work involves increased risk while driving during inclement weather to and from clients' homes and other work assignments.
  • Some irregular hours may occur and adjustments made to work assignment for the welfare of the patient.
  • Increased exposure to infectious diseases; must follow established protocols.

Qualifications:

  • 1 Year Homecare experience preferred.
  • Must possess excellent interpersonal skills.
  • Position requires a High School Diploma or GED and completion of a course of study approved by the NYS Education Department as qualifying for Licensed Practical Nurse.
  • LPN license and current registration is required.
  • Must pass a criminal background check, provide proof of a valid NYS Drivers License and current auto insurance certificate.
  • Must have reliable vehicle for transportation to and from work assignments.
  • Be knowledgeable in current routine nursing practices and demonstrate the application of those practices, procedures and techniques.
  • Ability to lift and transfer patients using appropriate body mechanics and lifting equipment.
  • Working knowledge of computers and other office equipment.
  • Ability to prioritize, organize tasks, and meet deadlines.

Job Type: Full-time

Pay: $24.04 to $32.21
