Job Summary:
The Licensed Practical Nurse (LPN) is responsible for providing high-quality, compassionate care to residents in a long-term care facility. Working under the supervision of registered nurses (RNs) and physicians, the LPN will ensure that residents health care needs are met and will assist in maintaining a safe, clean, and supportive environment for all residents.
Key Responsibilities:
- Resident Care:
- Provide direct nursing care to residents in accordance with established care plans, policies, and procedures.
- Administer medications, treatments, and other interventions as prescribed by the physician.
- Monitor residents for any changes in health status and report significant findings to the RN and/or physician.
- Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and eating, when necessary.
- Provide wound care and other specialized treatments as directed.
- Documentation & Reporting:
- Accurately document nursing care provided, including vital signs, assessments, medications, and treatments.
- Report changes in residents condition and any concerns to the appropriate staff members.
- Maintain confidentiality of all resident information in accordance with HIPAA guidelines.
- Collaboration & Teamwork:
- Collaborate with RNs, physicians, and other healthcare professionals to ensure optimal care for residents.
- Participate in the development and revision of care plans for residents.
- Communicate effectively with residents, families, and staff to promote understanding of care needs and changes.
- Quality Assurance & Safety:
- Ensure that infection control protocols and safety standards are followed at all times.
- Monitor and maintain a clean, safe environment for residents, adhering to facility policies.
- Participate in facility quality improvement initiatives and follow regulatory guidelines for long-term care.
- Family Support:
- Provide education and support to residents' families regarding care plans, health status, and long-term care needs.
- Address concerns from families in a timely and professional manner.
Qualifications:
- Education & Certification:
- Completion of an accredited LPN program.
- Current, active LPN license in the state of [State].
- Current BLS (Basic Life Support) certification required; additional certifications may be preferred.
- Experience:
- Previous experience in a long-term care or skilled nursing facility is preferred.
- Experience working with elderly residents and individuals with chronic conditions is a plus.
- Skills & Abilities:
- Strong communication skills, both written and verbal.
- Ability to work independently and as part of a team.
- Strong attention to detail and organizational skills.
- Ability to remain calm in high-stress situations and prioritize tasks effectively.
Working Conditions:
- Work is performed in a long-term care facility environment.
- Physical demands include standing, walking, lifting, and assisting with resident mobility.
- May require working evenings, weekends, and holidays as needed.
